This form should be used by adults participating as chaperones/advisors at a youth ministry program, especially off church property.
Youth Medical Consent Form

This form should be kept on file for each youth participating in youth ministry activities. Copies should always be carried on field trips off church property. The form should be updated and replaced at least annually.

This form must be completed by anyone authorized on behalf of the parish or youth minister to transport youth to/from any event in a private vehicle. Proper insurance is required to transport youth in private vehicles. This form alone does not fulfill that requirement. Please see the form for more information.
